FirstChoice Family Care offers comprehensive lab services, including standard screening blood work, hormone levels, thyroid tests, cholesterol levels, diabetes testing, comprehensive pediatric screening tests, urine tests, strep screens, flu tests, cultures, and anything else our providers feel you need to keep you healthy or diagnose your condition.

Sports Physicals

Let our experienced team of providers perform your child's sports and school physicals.  We offer vision and hearing screening as indicated and can make sure your child is healthy and safe to participate in athletics.

Hospital Medicine

Dr. Tompkins holds admitting privileges to both Livingston Regional Hospital and Cookeville Regional Medical Center.  His relationships with the Emergency Medicine and Hospitalist groups at both hospitals allow him to work closely with your provider to determine the most appropriate facility to meet your inpatient medical needs, and in most cases arrange for you to be directly admitted to the hospital if indicated.  Most of the time, this will allow you to skip a long wait in the ER.  Dr. Shaffer has close ties with Vanderbilt University Medical Center's Monroe Carell Jr. Children's Hospital and works closely with all providers to ensure your child is cared for properly in the hospital if the need arises. Dr. Thacker holds admitting privileges to Livingston Regional Hospital and The Medical Center at Albany.  He makes inpatient rounds at both facilities to ensure outstanding continuity of care.

Meet Our Providers

James Tompkins, MD

Dr. Tompkins is a lifelong resident of Byrdstown.  He graduated from Pickett County High School, completed a Bachelor of Science degree in Chemistry from Tennessee Technological University, and received his Doctor of Medicine from  the James H. Quillen College of Medicine in Johnson City, Tennessee.  He then completed a residency in Family Medicine at the University of Tennessee Medical Center in Knoxville, Tennessee.  He is a Diplomate of the American Board of Family Physicians (ABFM), a Fellow of the American Academy of Family Physicians, and additionally holds the Designation of Focused Practice in Hospital Medicine from the ABFM.  Dr. Tompkins has practiced Primary Care in the Upper Cumberland area since 2010, as well as Hospital Medicine and Emergency Medicine.  He currently serves as Associate Medical Director for Caris Hospice, and is Medical Director for Pickett County Emergency Medical Services.  Additionally, he serves as Pickett County Medical Examiner and volunteers as Team Physician for Pickett County K-8 and High School.

Jori Shaffer, MD

Dr. Shaffer grew up in Byrdstown. She graduated from Pickett County High School in 1990, completed a Bachelor of Science degree in Chemistry from Tennessee Technological University in 1994, and received her Doctor of Medicine Degree from the University of Tennessee Memphis in 1998. She then completed a combined Internal Medicine and Pediatrics residency program at Vanderbilt University in Nashville, TN in 2002. She is a Diplomate of the American Board of Pediatrics. She has practiced medicine in Byrdstown, caring for patients from all over the Upper Cumberland and Southern Kentucky region, for nearly 18 years.
Dr. Jori (as her patients affectionately call her) resides with her husband Lance and their sons Quinn, Zeke and Zeb in Livingston, TN. They live on a small farm and raise alpacas. She and her husband are active in their local church, especially in the worship ministry. They all love music and doing family activities and adventures together.

Thomas Thacker, DO

Dr. Thacker is originally from Albany, Kentucky.  He graduated from Clinton County High School in Albany, Kentucky. He earned an Associate's Degree in Health Science from Lindsey Wilson College in Columbia, Kentucky, and a Bachelor of Science Degree in Biology and Chemistry from Western Kentucky University in Bowling Green, Kentucky. Additionally, he holds a Master of Science Degree from the University of Kentucky in Lexington, Kentucky and received his Doctor of Osteopathic Medicine from the University of Pikeville's Kentucky College of Osteopathic Medicine. He then completed a Family Medicine residency at the University of Tennessee-Nashville (Ascension-Saint Thomas Health System). He is a Diplomate of the American Board of Family Physicians. Dr. Thacker has been a practicing medical professional since 2008. He practiced as a Physician Assistant in Glasgow, Kentucky and Munfordville, Kentucky prior to medical school. After completing his Family Medicine residency, he chose to serve the Dale Hollow Lake area and surrounding communities. He understands that your health and wellness concerns extend beyond routine medical care. He is a highly trained, board-certified Family Physician and provides an array of comprehensive services and procedures within his field.

Beth Crabtree, PA-C

Beth grew up in Albany, Kentucky and graduated from Clinton County High School in 2002. She completed her Bachelor of Science degree in Emergency Medical Care at Eastern Kentucky University in 2006. She worked as a Paramedic while completing her Masters in Science in Physician Assistant Studies at the University of Kentucky. Beth has been a Physician Assistant for twelve years, with work experience in Emergency Medicine, Urgent Care, and Primary Care. She resides in Pickett County with her husband, Steve, and four children: Kaylin, Samuel, Isabella and Charlee Grace. Her favorite things to do are reading, running and relaxing on Dale Hollow Lake.
